Dental Treatment Plan Policy 

Effective Date: 13/06/2024 

Expiry Date of Treatment Plans: 3 Months from Date the Treatment Plan was Issued. 

 

Overview 

This policy outlines the framework for dental treatment plans issued by Johnstown Dental Practice. Each treatment plan provided to patients will be valid for a period of three months from the date the treatment plan was created. This ensures that the patient receives timely and relevant dental care, considering any changes in their oral health. 

Purpose 

The purpose of this policy is to: 

  • Ensure patients receive appropriate and timely dental care. 

  • Maintain up-to-date and relevant treatment plans. 

  • Encourage patients to complete their treatment plans within a reasonable timeframe. 

Scope 

This policy applies to all dental treatment plans issued by Johnstown Dental Practice, including but not limited to general dentistry, orthodontics, periodontics, endodontics, and cosmetic dentistry. 

Policy Details 

1. Treatment Plan Validity 

  • All treatment plans are valid for a period of three months from the date of issuance. 

  • The expiry date will be clearly stated on the treatment plan. 

2. Issuance of Treatment Plans 

  • Treatment plans will be issued after a comprehensive dental examination. 

  • Each plan will include a detailed description of recommended treatments, estimated costs, and the expected duration of the treatments. 

3. Review and Updates 

  • Patients are encouraged to review their treatment plans and discuss any concerns with their dentist as soon as possible. 

  • If a patient's oral health condition changes within the validity period, the treatment plan will be reviewed and updated accordingly. 

4. Extension of Treatment Plans 

  • Extensions beyond the three-month validity period may be granted in exceptional circumstances, subject to the approval of the treating dentist. 

  • Patients must request an extension before the expiry date of the current treatment plan. 

  • Extensions will be considered on a case-by-case basis and will not exceed an additional three months. 

5. Expired Treatment Plans 

  • Once a treatment plan expires, a new comprehensive dental assessment will be required to issue a new treatment plan. 

  • The new plan may include revised treatment recommendations based on the current oral health status of the patient. 

6. Communication and Documentation 

  • All treatment plans and any changes or extensions will be documented in the patient’s dental records. 

  • Patients will receive a copy of their treatment plan and any subsequent updates or extensions. 

7. Financial Considerations 

  • The estimated costs provided in the treatment plan are valid for the three-month period. Changes in costs may occur if the treatment plan is updated or extended. 

  • Patients are advised to discuss payment options and insurance coverage with the clinic's Practice Manager. 

Responsibilities 

Dentists 

  • Conduct thorough examinations and provide accurate treatment plans. 

  • Communicate clearly with patients about their treatment options and the validity period of the plan. 

  • Update and review treatment plans as needed. 

Patients 

  • Review their treatment plans and adhere to the recommended treatment schedule. 

  • Communicate any changes in their oral health or concerns about the treatment plan promptly. 

  • Request extensions, if necessary, before the expiry date. 

Compliance 

Compliance with this policy is mandatory for all staff and patients of Johnstown Dental Practice. Failure to adhere to this policy may result in the need for a new examination and treatment plan, potentially delaying necessary dental care. 

Review 

This policy will be reviewed annually and updated as needed to ensure continued relevance and effectiveness.
